Limb Preservation Surgery

Limb preservation (salvage) surgery aims to remove malignant bone or soft tissue tumors while preserving the limb’s function and appearance, avoiding amputation in over 90% of cases today. Performed after neoadjuvant chemotherapy, it involves wide tumor resection with clear margins, followed by reconstruction.

Types of Reconstruction:

Endoprosthesis (Megaprosthesis): Custom or modular metal implants replace resected bone/joint, ideal for peri-articular tumors.

  • Biological Reconstruction: Allografts (donor bone) or vascularized fibula grafts for better long-term integration.
  • Recycled Autograft:
    • Cryotherapy: Resected bone frozen in liquid nitrogen to kill cells, then reimplanted.
    • ECRT: Bone irradiated extracorporeally (50 Gy) before reuse.
